Care at Home – Torbay and South Devon NHS Foundation Trust

Accessible healthcare

Oftentimes, people with complex medical needs have found the current system difficult to navigate, resulting in delays in care and miscommunication between healthcare professionals and their patients. However, the advancement of technology has given way to many possibilities in healthcare, particularly for those who need care at home.

For Torbay and South Devon NHS Foundation Trust, one such instance arose when a house bound patient in a wheelchair with multiple medical conditions required a complex case shared decision meeting with professionals and clinicians. He had been in and out of hospital over a period of several months and his situation was made all the more difficult due to living in a first floor flat, meaning mobility was limited. Due to the organisational restrictions, he was unable to have a clear direction of care and had been waiting for over a year before a new solution was created to enable the MDT meeting to take place.

Innovating care

Currently, a traditional system still operates in NHS hospitals where a patient attends only one appointment for one consultant, meaning there is very little to no capability to allow multiple specialities within the communities, carer’s and the acute setting to work as MDTs. Communication between the patient, their carer’s and their clinicians becomes difficult and any input into the decision-making process on the patients side becomes minimal.

In the modern era, it has become outdated and impractical for disabled and vulnerable patients particularly as this process has led to too many patients possibly being sent to the wrong consultant, delaying medical care. However, with  the advent of video conferencing, it creates a new model of care, particularly for those who are housebound with complex health issues and a need for input from multiple healthcare professionals.

Visionable was able to help transform the way care and medical decisions were made. Creating new and more modern forms of communication meant that MDT meetings could take place within a patients home. Because of Visionable, a GP, Nurse, Consultant Urologist, Consultant Anaesthetist and Intensive Care Rehabilitation Co-ordinator and Specialist Occupational Therapist were able to meet to have a fully interactive shared decision-making consultation remotely.

Through the virtual consultation, Visionable was able to hand back control over to the patient and allow him to ask questions to all the care providers in a calm environment. The multidisciplinary team were able to question each other in real time to conclude the best way forward and give the patient full information and participation in the decisions being made and agree on a new care pathway which he felt was best for him. The process solved the problems of having catheters’ replaced at home without a hospital visit and care providers could stay local, to their place of work, eliminating the need for travel and the associated time attending the meeting.
